Step One: Lighthouse Professional Placement & Caregivers will complete a comprehensive intake form for your loved one and assess all of their needs to help determine the best type of community and level of care he/she would need.

Step Two: Once you have chosen your geographical area, our next step is to use your personalized assessment information to select the top rated, licensed communities that will provide the environment and quality care that is essential to our client’s comfort. Together with the family, we will go over all of the advantages of each community that we’ve found and determine which one/s would be best. Then, with your permission, we forward the personalized assessment to your chosen providers and set up a tour of the facilities. We can take you on this tour to answer any questions you may have and help facilitate or, if you prefer, we can provide you with the list for you to follow up with.

Step Three: Now that you’ve toured the communities and have come to a decision on a community, Lighthouse Professional Placement & Caregivers will then negotiate all of the fees, coordinate completion of the required Physicians Report and schedule a move-in date. In addition to this, we will facilitate the transition and complete all the necessary paperwork with the desired community to ensure that you get the best terms and receive the proper level of care needed. We will coordinate with the Family, Hospital, social worker and/or case manager to ensure a seamless transition to your new community. We can also arrange for medical transportation for your loved one to the community.

Step Four: Lighthouse Professional Placement & Caregivers will follow up with the resident periodically after placement, to ensure that all is going well with the resident in their new home. We pride ourselves on our post placement follow up and our on-going, extensive community screening process. Please note that we provide all of these services with no obligation and no cost to you.
